Clover Park School District (CPSD) is proud to partner with the Military Adolescent School-Based Health Initiative (MASBHI) to provide Teen Wellness and Health Service Clinics at Lakes High School, Woodbrook and Mann Middle Schools and Harrison Preparatory School.

All Defense Eligibility Enrollment Reporting System (DEERS) enrolled active duty or retired dependent teenagers are eligible for health care services in the schools.

CPSD and Madigan Army Medical Center (MAMC) are committed to maintaining a partnership that ensures easy access to health care for all eligible military associated students. By operating a health center on school grounds, MAMC accepts a unique responsibility to promote a safe and healthy school environment for all students. MASBHI staff will cooperate and communicate with CPSD staff whenever student behavior or health may result in risk of harm to the student or others within the educational setting. The MASBHI staff will follow policies and procedures developed by MAMC as well as those detailed in CPSD board policies and procedures.

If you anticipate that your teen will receive an influenza immunization this season, please fill out and sign the Influenza Immunization Screening Form and send it to your teen's appointment. Please call the Adolescent Medicine Division at Madigan at 253- 968-1980This form must be completed and signed by a parent for the student to receive an immunization.

Consent must be signed to receive school-based health services. Forms may also be accessed at the Madigan Adolescent Clinic, OR by registering at www.relayhealth.com and requesting a consent form to be emailed to you directly. Mention school-based health in the message to office staff.
