Waivers

Student Services manages the waiver process, including Application for Non-Resident Admission, Request for Transfer Between Clover Park Schools, and Request for Release of Attendance forms. All forms are available for download, below.

These forms can also be picked up in the Student Services Department office Monday through Friday, 7:30 am to 4:30 pm. We do close for lunch from 12:30 pm to 1:30 pm. Waiver Forms and Procedures

Application for Non-Resident Admission (Español)– Parents can pick this up at the school they wish their child to attend. Parent completes the form, and takes it to the desired school, along with a copy of the Release of Attendance form from the resident school district. The school will tell the parent if their student is accepted or not. If the parent does not know where to obtain a Release of Attendance form from their resident school district, see our Release of Attendance List (pdf).

Between Schools Transfer (Español)– Parents can pick up this form up at any building, or at the Student Services Department. Parent completes the form, and takes it to the school they wish to be released from. The releasing school will approve or deny the release. If approved, the parent can then take the Transfer to the school they wish to attend, or the releasing school can forward it for them. The receiving school will notify the parent if their child is accepted into that school or not.

Request for Release of Attendance (Español) – These are only available from the Student Services Department. Parent or adult student completes the form and returns it to the Student Services Department. Staff will review for completeness, sign off on the approval, and make 2 copies. Both copies are given to the parent/adult student – 1 is for the new school, the other is for the parent/adult student's records. Students 18 and over may sign their own documents.
